Arizona verification steps<\/h2>\n

Step 1: Name the responsible clinician and credential<\/h3>\n

Ask for the full legal name and credential of the person who will review your history, evaluate you, and make the prescribing decision. In Phoenix that may be a naturopathic physician (NMD), a medical doctor (MD) or doctor of osteopathy (DO), a nurse practitioner, or another licensed professional. Each credential has a different Arizona board. A clinic brand, a medical-director name, or the phrase “medical team” is not an answer.<\/p>\n

Step 2: Search the correct Arizona board<\/h3>\n

For a naturopathic physician, use the Arizona Naturopathic Physicians Medical Board public register<\/a>. Search an MD through the Arizona Medical Board’s Find Your MD<\/a> directory. Search a DO through the separate Arizona Board of Osteopathic Examiners Find Your DO<\/a> directory. For an RN or nurse practitioner, use the Arizona Board of Nursing’s primary-source verification route<\/a>, which directs RN, LPN, and APRN searches to Nursys. These public profiles establish the board’s current record; they do not rate care quality.<\/p>\n

Match the name, credential, license number, status, and expiration to what the clinic supplied. A name match with the wrong credential, or an expired record, belongs in the Mismatch<\/strong> column until the clinic explains it with a current public record.<\/p>\n

Step 3: Read the public action route<\/h3>\n

Keep the action history as its own check. Record the “public board actions” field the register or board profile returns for the clinician, and write down the date you searched. A blank field does not prove a clean history. Record what the source displayed on that date. A complaint page tells you where a board accepts information; it does not establish that misconduct occurred.<\/p>\n

Step 4: Cross-check the NPI without treating it as a license<\/h3>\n

Search the clinician or organization in the CMS NPI Registry<\/a>. An NPI is a national identifier used for covered health care providers. It can corroborate a name, taxonomy, and practice address, but it is not a license, a disciplinary clearance, or an endorsement. Label it not a license<\/strong> in your file, and use the correct Arizona board for the license itself.<\/p>\n

Step 5: Distinguish a pharmacy person-license from a facility permit<\/h3>\n

Ask which pharmacy or dispensing source will appear on your label, and get its exact legal name and physical location. Arizona separates a pharmacist’s individual license from a pharmacy or facility’s permit. The Board of Pharmacy’s license and registration<\/a> page and its online license lookup<\/a> are primary-source routes; the Board states the online result is primary-source verification. Verify the exact legal entity, and use the Board’s permit verification route<\/a> for a dispensing pharmacy or facility. A trade name without a legal entity can send you to the wrong record, and a registration is not FDA approval.<\/p>\n

Step 6: Separate 503A and 503B facts<\/h3>\n

Section 503A and Section 503B describe different federal conditions. Section 503A covers conditions that may apply when a licensed pharmacist compounds in a state-licensed pharmacy or federal facility, or when a physician compounds; FDA maintains bulk-substance lists<\/a> under that section. Section 503B covers outsourcing facilities<\/a> that register with FDA and face a different oversight structure. FDA states that compounded drugs are not FDA-approved.<\/p>\n

If a clinic names a 503B source, search the exact facility on FDA’s registered outsourcing facilities page and record the registration date, last inspection, any Form 483, any recall field, and the action based on the last inspection. Registration is not approval and does not replace current inspection or action review. FDA’s BeSafeRx<\/a> page is a routing aid to the state board, not an endorsement list. Step 7: Make the product answer specific<\/h3>\n

Ask the clinic to name the exact drug or substance, formulation, proposed use, and whether the product is FDA-approved for that use, used outside an approved label, compounded, or investigational. A broad claim about “peptides” cannot be checked. Ask the clinician to explain the reason for the proposed option, the known risks, the alternatives, and the limits of the evidence for a person with your health history, and keep that answer in your file.<\/p>\n

Step 8: Put price, monitoring, and records in writing<\/h3>\n

Ask for one written quote that separates first-month, recurring, one-time, conditional, cancellation, and records charges. Leave blank any item the clinic has not answered. Then name the person responsible for follow-up: how you reach them, which monitoring is expected, what symptoms or problems should trigger contact, how the clinic handles an adverse event, and how you obtain your records.<\/p>\n

This dated example shows how the checks work on public records. It does not recommend the clinic or make a claim about treatment quality, current service availability, product status, safety, effectiveness, or suitability.<\/p>\n

On August 27, 2026<\/strong>, we read the Arizona Naturopathic Physicians Medical Board public register for “Greg Jones” as part of the Phoenix directory. The register returned a naturopathic medical doctor license 19-1774<\/strong>, active with an expiration of 2027-01-01<\/strong>, and a separate dispense-for-profit record active through 2027-07-01<\/strong>. Both listed no current public board actions on that date. The register also returned an expired preceptorship-training record (23-3185<\/strong>, expired 2024-07-01); the current NMD license, not the expired training record, is the relevant clinician credential.<\/p>\n

NPPES NPI 1598226425<\/strong> corroborated the name Greg Jones, NMD and the clinic address. The first-party Enovative Wellness Center page names Greg Jones, NMD as medical director and lists 4425 N. 24th St., Suite 100, Phoenix, Arizona 85016. Official January 14, 2021 Board minutes state that Case 20-166 involving Greg Jones was dismissed after the Board found no evidence supporting the allegation.<\/p>\n

These identity and licensure facts reflect the retrieval date. They establish active licensure and NPI corroboration for one directory listing. They do not establish product status, service availability, treatment quality, safety, effectiveness, or suitability. Public records change, so re-run each field when you compare the clinic.<\/p>\n

Save a record, not a score<\/h2>\n

Keep the clinic’s written answers, the public URLs, the date you searched, and a screenshot or PDF of each result. Public databases change, and your saved evidence shows what you reviewed and what remained unclear on that date.
